Participant Waivers
All participants must complete the required waivers before their first day of play at Cherokee Homeschool Hoops.
Important: Two Waivers Required
Children must complete both the Homeschool Hoops Waiver (presented by Shot Scan Basketball) and the Waiver for your Basketball Facility before participating.
Homeschool Hoops Waiver

This digital waiver covers participation in all Cherokee Homeschool Hoops activities, including practices, games, and events. Complete this waiver online — it only takes a few minutes.
Facility Waiver
Required by our host facilities
Some host facilities require their own liability waiver. Please complete the waiver for the location where your child will be participating if applicable.
Calvary Baptist Church Waiver

Print this waiver and bring it completed on your first day. Extra copies will be available if forgotten, but completing it ahead of time helps us get started faster.

No facility waiver required. Trinity Presbyterian does not require a separate facility waiver. You only need to complete the Homeschool Hoops waiver above.
Waiver Checklist
- Complete the Homeschool Hoops online waiver — required for all participants
- Print and complete your facility waiver — based on your session day (Wednesday or Friday)
- Bring the printed facility waiver to your first session
